Terms & Conditions
Effective Date: September 2026
Website: https://cvk3.com
Please read these Terms and Conditions carefully before using the CVK Precision Health website or services. By accessing our website, submitting an intake, booking a consultation, or using our services, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.
1. About CVK Precision Health
CVK Precision Health ("CVK," "we," "us," or "our") provides physician-guided precision health, wellness, and health optimization services primarily through telehealth.
Our services may include health assessments, laboratory coordination, biomarker reviews, physician consultations, personalized wellness protocols, health coaching, ongoing monitoring, and access to medications or supplements when determined clinically appropriate by a licensed provider.
Our website provides information about our programs and allows eligible individuals to request consultations and related services.
2. Medical Disclaimer
The general content available on our website is provided for informational and educational purposes only. It is not intended to replace professional medical advice, diagnosis, or treatment.
Clinical services offered through CVK are provided or supervised by licensed healthcare professionals. No provider-patient relationship is created simply by visiting the website, submitting a form, joining a waitlist, or requesting information.
A provider-patient relationship may only be established after completion of the required intake process, consent forms, identity and location verification, and consultation with an appropriately licensed healthcare provider.
Always consult a qualified healthcare provider regarding any medical condition, symptoms, medications, or treatment decisions.
If you are experiencing a medical emergency, call 911 or visit the nearest emergency room immediately.
3. Eligibility
By using our website or services, you represent that:
- You are at least 18 years old.
- You have the legal capacity to enter into a binding agreement.
- You will provide accurate and complete information.
- You are physically located in a state where the applicable provider is licensed.
- You will use our website and services only for lawful purposes.
CVK services may not be available in every state. We reserve the right to decline services if you do not meet legal, geographic, or clinical eligibility requirements.
4. Appointments and Consultations
a. Booking
Appointments may be requested through our website, scheduling platform, telephone, email, or another approved method.
Submitting a booking request does not guarantee that an appointment or service will be provided. We reserve the right to decline, cancel, or reschedule appointments based on provider availability, eligibility, safety, or clinical considerations.
b. Cancellations and No-Shows
A $25 no-show fee applies to missed appointments. A missed appointment is not the same as a cancellation: cancellations made before the appointment are governed by the cancellation terms in your booking confirmation, while a no-show relates to an appointment that was not attended.
Rescheduling depends on the service you purchase:
- General Consultation ($149): no rescheduling is permitted. If you miss the appointment, you must book and pay for another consultation.
- Plans below $200: no rescheduling is permitted. If you miss the appointment, please follow the applicable rebooking process.
- Plans that include laboratory testing: one-time rescheduling is permitted.
The applicable policy is shown on the service page and on the booking page before you confirm your appointment.
c. Appointment Preparation
You may be required to complete intake forms, provide identification, disclose your current location, submit medical records, or complete laboratory testing before your consultation.
Late arrivals may result in a shortened appointment, cancellation, or rescheduling.
5. Telehealth Services
Some CVK services are provided through video calls, telephone calls, electronic forms, secure messaging, or other telehealth technologies.
By participating in telehealth services, you understand that:
- Telehealth may have limitations compared with an in-person examination.
- Technical issues may interrupt or delay a consultation.
- A provider may require additional testing or an in-person evaluation.
- You are responsible for using a private location and secure internet connection.
- You must not record consultations without prior permission.
A separate telehealth consent form may be required before services are provided.
6. Payments and Fees
Payment may be required before a consultation, laboratory service, assessment, product, or other service is provided.
Consultations and services are purchased individually. A purchase covers the selected consultation or service; it does not create a recurring membership or an automatic renewal.
By providing payment information, you authorize CVK and its payment processors to charge the applicable fees.
Prices may vary depending on your selected program, physician recommendations, laboratory requirements, medications, supplements, and other services. Prices may change for future services without prior notice.
You are responsible for any fees charged by your bank, credit card provider, financing provider, or payment processor.
Refunds, where available, will be governed by the refund or cancellation policy disclosed at the time of purchase.
7. Insurance
Unless expressly stated otherwise, CVK services may not be covered by health insurance, Medicare, or Medicaid.
You are responsible for confirming whether your insurance provider offers reimbursement for consultations, laboratory testing, prescriptions, or other services.
CVK does not guarantee insurance coverage or reimbursement. You remain responsible for all applicable fees regardless of whether your insurance provider reimburses you.
8. Health Information and Informed Consent
Before receiving services, you may be required to:
- Complete a health and medical-history questionnaire.
- Disclose current medications, supplements, allergies, and health conditions.
- Provide laboratory results or other relevant records.
- Review and sign informed-consent documents.
- Confirm your identity and physical location.
You represent that all information you provide is truthful, current, and complete.
Incomplete or inaccurate information may affect the safety and effectiveness of your care. CVK is not responsible for outcomes caused by information that you intentionally fail to disclose or provide inaccurately.
9. Laboratory Testing and Home Phlebotomy
CVK may coordinate laboratory testing, home blood collection, specimen collection, or related services through independent laboratories and licensed service providers.
You are responsible for following all preparation, collection, storage, and shipping instructions.
Testing or processing may be delayed due to insufficient samples, collection errors, shipping delays, laboratory issues, weather, or circumstances outside our control.
Laboratory results should not be used for self-diagnosis or independent changes to your medications or treatment plan. Results should be reviewed with a qualified healthcare provider.
10. Prescriptions and Clinical Decisions
All clinical decisions are made by licensed healthcare providers using their independent professional judgment.
Completing an intake or consultation does not guarantee that you will receive a prescription, medication, supplement, laboratory order, or particular treatment recommendation.
A provider may determine that:
- A requested service is not clinically appropriate.
- Additional testing or records are required.
- An in-person evaluation is necessary.
- You should seek care from another healthcare provider or specialist.
Any prescription medication must be used only as directed by the prescribing provider and dispensing pharmacy.
11. Compounded and Off-Label Medications
Certain medications recommended through CVK may be compounded by licensed pharmacies or prescribed for an off-label use when a licensed provider determines that doing so is clinically appropriate.
Compounded medications are not reviewed or approved by the FDA in the same manner as commercially manufactured, FDA-approved medications.
The prescribing provider or pharmacy will provide applicable information regarding potential benefits, risks, alternatives, dosage, and proper use.
12. No Guarantees of Results
Individual results vary based on many factors, including age, medical history, genetics, lifestyle, nutrition, physical activity, sleep, health conditions, medication use, and adherence to provider recommendations.
CVK does not guarantee any specific health, wellness, performance, weight, longevity, laboratory, or treatment outcome.
Testimonials, reviews, and case studies represent individual experiences and are not necessarily typical. They do not guarantee that you will achieve the same or similar results.
13. Third-Party Providers
CVK may use or coordinate services with independent laboratories, pharmacies, phlebotomists, technology providers, scheduling platforms, payment processors, delivery companies, or other third parties.
These providers may have their own terms, privacy policies, fees, processing times, shipping policies, and eligibility requirements.
CVK is not responsible for the actions, availability, delays, errors, or services of independent third-party providers where those matters are outside our reasonable control.
14. Intellectual Property
All content available on the CVK website, including text, logos, graphics, videos, images, designs, questionnaires, software, educational materials, and branding, is owned by or licensed to CVK Precision Health and is protected by applicable intellectual-property laws.
You may use website content only for personal and non-commercial purposes.

Our total liability for any claim arising from these Terms or your use of our services shall not exceed the amount you paid to us in the twelve (12) months preceding the claim.
17. Indemnification
You agree to indemnify and hold harmless CVK Precision Health and its affiliates, officers, employees, providers, and partners from any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms, your misuse of our services, or your provision of inaccurate or misleading information.
18. Disclaimer of Warranties
